Facility Patient Advocate Specialist

What You’ll Do
As a Facility Patient Advocate Specialist, you’ll represent HPG at your assigned facilities by supporting patients, families, caregivers, and providers. Your role focuses on communication, coordination, and ensuring patients feel supported between provider visits. Daily responsibilities may include:
- Rounding in facilities to collaborate with staff and check on patients
- Building strong relationships with patients, families, and caregivers
- Relaying time-sensitive concerns or updates to providers
- Assisting patients with scheduling specialist appointments
- Serving as the primary point of contact for patient support and continuity of care
This role offers a dynamic, people-centered environment where no two days are the same.
What You Bring
- High school diploma or GED
- Certified healthcare credential (CMA, RMA, MA, EMT, CNA, or similar)
- Experience in a medical office or healthcare setting
- Strong communication and interpersonal skills
- Proficiency in Microsoft Word, Excel, and Outlook
- Confidence in using electronic records and documentation
- Excellent organizational skills and attention to detail
- Ability to think critically and problem-solve in real time
- Reliable transportation to travel to assigned facilities
