Our company was founded in 2005 and serves the Central Florida Geriatric population. We are a large practice with over 25 providers that serve Skilled Nursing Facilities, Assisted Living Facilities and our homebound patients in the comfort of their home in 14 counties. We are committed to providing excellent care to our patients and buildings while reducing hospital readmissions.

Facility Patient Advocate Specialist

Orlando • Sanford • Central Florida

What You’ll Do

As a Facility Patient Advocate Specialist, you’ll represent HPG at your assigned facilities by supporting patients, families, caregivers, and providers. Your role focuses on communication, coordination, and ensuring patients feel supported between provider visits. Daily responsibilities may include:

  • Rounding in facilities to collaborate with staff and check on patients
  • Building strong relationships with patients, families, and caregivers
  • Relaying time-sensitive concerns or updates to providers
  • Assisting patients with scheduling specialist appointments
  • Serving as the primary point of contact for patient support and continuity of care

This role offers a dynamic, people-centered environment where no two days are the same.


What You Bring

  • High school diploma or GED
  • Certified healthcare credential (CMA, RMA, MA, EMT, CNA, or similar)
  • Experience in a medical office or healthcare setting
  • Strong communication and interpersonal skills
  • Proficiency in Microsoft Word, Excel, and Outlook
  • Confidence in using electronic records and documentation
  • Excellent organizational skills and attention to detail
  • Ability to think critically and problem-solve in real time
  • Reliable transportation to travel to assigned facilities
